  • Description

The Originating Agency Identification (ORI) number for APD CDC is EAPDFC20Z. You'll need this number if you're applying for a role under the Consumer-Directed Care (CDC) program with the Agency for Persons with Disabilities (APD) or if your employer requests live scan fingerprints for regulatory compliance.

Who Needs to Use This ORI Number?

If you are applying for or renewing any of the following positions, you may need to submit fingerprints using this ORI number:

  • Caregivers for Individuals with Disabilities
  • Personal Support Workers under the CDC Program
  • Family Members Providing Direct Care in the CDC+ Program

How to Get an ORI Number?

  • If your employer or regulatory agency requests live scan fingerprints, they may provide you with an ORI number.
  • If you're applying for a position, contact the Florida Agency for Persons with Disabilities (APD) Consumer-Directed Care Program for guidance.
  • Check your documentation from your employer, such as an email or appointment sheet. If you're still unsure, contact your HR department or licensing authority.

Background Check

You'll need to pass a background check and meet the regulatory requirements for working under the CDC+ program. If you have a criminal history, your application will be reviewed to determine if you meet the eligibility requirements.

How to Submit Fingerprints

  1. Use a Live Scan Provider – Fingerprints must be taken electronically through an approved FDLE (Florida Department of Law Enforcement) Live Scan provider.
  2. Provide the ORI Number – When booking or at the appointment, ensure you inform your fingerprint technician of the ORI number (EAPDFC20Z) to ensure the results are correctly sent to the Florida APD CDC+ Licensing Division.
  3. Schedule an Appointment – You can book an appointment with Miami Live Scan or another authorized provider online.

Confirming Receipt

After submission, check with the Florida APD Consumer-Directed Care Program to confirm they received your fingerprint results. Using your TCN (Transaction Control Number) or Appointment ID, you can check the status at:
